Doctors, pharmacists, nurses: easily share medication reviews
Objectives, content, billing: everything you need to know about this new nursing procedure for non-addictive multi-medication patients.
January 2022 will see the inclusion in the NGAP of a new medico-nursing act, "Accompagnement par les infirmiers de la prise médicamenteuse à domicile". This initiative is part of the evolution of nurses' missions, particularly with regard to the clinical monitoring and therapeutic education of patients with chronic pathologies, in coordination with the attending physician.
Objectives
- Promote treatment compliance,
- Improving medication compliance,
- Prevent the risks associated with drug iatrogeny and inappropriate hospitalization.
Prescription
This nursing support will be prescribed by the physician:
- when implementing a treatment
- when modifying a treatment
- during a clinical situation likely to call into question the therapeutic strategy
For non-dependent, multi-medicated patients presenting criteria of frailty identified by the attending physician or prescriber.
This support will include 3 sessions within a maximum of one month, renewable on prescription once within the following 12 months.

The first session would be valued at AMI 5.1.
The next two sessions at AMI 4.6.

- Monitoring: objective clinical data including pulse, BP, weight, etc.
- Observation: recording of clinical and subjective elements, including complaints, behavior, etc.
- Verification of: patient and/or family understanding of prescription(s), search for co-prescribers
Implementation
- Check that the pillbox has been prepared according to the current prescription(s),
- Medication and time constraints, in relation to meals, according to food...
- Management of medication stock according to the prescriber(s) (accumulation of medication, expired medication, search for self-medication, etc.).
Evaluation and reporting
- Search for reasons for not taking medication or for changes in dosage
- Looking for side effects
- Collection of social context factors that may affect compliance
- Search for explanations for non-adherence during treatment, according to the doctor's specific request
