Chloé is the private nurse for Paul, 72, who lives alone in his own home. Depressed, he suffers from heart failure with atrial fibrillation and osteoarthritis of the knee requiring analgesic treatment.
A fragile, multi-medicated patient, his medication is delivered in a secure Medipac pillbox by his pharmacist, who prepares the doses to be administered.
Without a family environment, he relies on an auxiliaire de Vie Sociale every day to run errands and help prepare meals. Chloé comes to his home every week to provide clinical supervision.
The Medissimo Nurse solution solution enables nurses to monitor by nurses, to help ensure compliance and inform healthcare professionals in the event of risks. It is free to use.
With the Medissimo Infirmière application, Chloé synthesizes the monitoring data she collects in the form of a medication report in Paul's DMP. This real-time sharing of information with the doctor and pharmacist enables us to assess Paul's actual level of adherence and tolerance to his treatment. This enhanced cooperation means that Paul's doses of certain medications, whose useful doses are close to dangerous ones, can be adapted more reactively.
Chloé registers with Medissimo Infirmière and asks Paul's pharmacist to activate the connection. At each visit, Chloé can enter real-time information on the medication taken, qualify non-compliant doses, record adverse events and measure the level of discomfort felt by Paul, report events leading to dose adjustments and specify additional medication taken.
Chloé and Paul's pharmacist can exchange information directly via the app, relieving Paul and his caregivers of the constraints associated with prescription renewals, changes to his treatment and dispensing procedures.
The data entered by Chloé feeds a medication report into the application, which encourages dialogue and cooperation, as it is shared with the pharmacist and the doctor via Paul's DMP to review his treatment.
Paul's initial assessment showed that he was not adhering sufficiently to his diuretic treatment. The clinical observations recorded by Chloé enabled us to identify predictive signs of decompensation of his heart failure, thus avoiding emergency hospitalization.
Supported by a medication check-up, Chloé suggested to Paul's doctor that she intensify her treatment and visit Paul once a day, just long enough to balance his medication.
