Doctors, pharmacists, nurses: easily share medication reviews
This is the day-to-day task of Bénédicte, health care manager at EHPAD Les Myrtilles: optimizing medication distribution time and avoiding errors. For some time now, Bénédicte has been noticing organizational shortcomings that are complicating her mission and wasting her time.
Bénédicte chases prescriptions and medication. Treatment changes and emergency management are becoming increasingly disorganized. Bénédicte has the impression that there are shortcomings in the medication circuit.

The solution Mono Multi solution includes a pill dispenser secured by a pharmacist's application, which is connected to a care management software package. The solution can integrate an administration traceability application. The Mono Multi solution makes it easy to track prescription, dispensing and administration data, as well as iatrogenic risk prevention indicators, enabling treatment to be adjusted for better long-term health outcomes. The doctor exports his prescription to the pharmacy, the pharmacist dispenses treatments in pillboxes and exports treatment content to the facilities, and the nurse delegates administration and feeds it into reports, enabling treatments to be revised.
First of all, Bénédicte carries out an initial diagnosis with a digital audit of the medication circuit, based on 100 questions. This multi-disciplinary approach brings together all EHPAD and dispensary professionals.
Based on the results, Bénédicte convened a meeting of the care team, who decided to interoperate the software and organize the distribution of medicines in carts, in the form of named boxes containing pill doses and medicines remaining in their boxes.
Bénédicte and her manager make an appointment with the pharmacy to propose these improvements and validate them by signing the Medissimo EHPAD-Officine agreement. The director and the pharmacist agree on how the improvements will be financed.
The EHPAD and Officine teams are installing their own software interfaces, so they no longer have to chase prescriptions and their treatments are always up to date.
For greater peace of mind, our teams have set up a kit for urgent care needs on a doctor's prescription, renewed as and when needed.
Receiving chronic and intercurrent prescriptions at the pharmacy every Wednesday enables the pharmacist to prepare and dispense treatments every Friday.
It's much simpler for the EHPAD team, who receive a week's worth of medication and organize the cart for Monday.
The simplicity of distribution in the form of daily name-boxes ensures that temporary workers are quickly trained and efficient.
Bénédicte can see that her organization is improving and that the pharmacy is satisfied. She recovers nursing time, as she no longer feels the need to intervene all day to resolve medication problems.
To ensure that this continues, the care team decides to renew the diagnosis every 6 months and to organize a meeting with the pharmacy.
